What FDA Inspectors Actually Look for in GMP Training Records — And How AI Is Changing What's Possible
GMP training deficiencies are a perennial FDA 483 citation. Here's what inspectors check under 21 CFR 211.25 — and how AI closes the gaps before they do.
Training records are where FDA investigators find the gaps you didn’t know you had. Personnel qualifications and training deficiencies — governed primarily by 21 CFR 211.25 for finished drug manufacturers — have appeared on FDA’s published list of the most frequently cited Form 483 observations for more than eight consecutive fiscal years. That’s not an anomaly. It’s a signal that most regulated facilities still treat training as a documentation exercise rather than a compliance control — and inspectors know exactly where to look.
This piece is for quality directors, VP-level compliance leads, and anyone who owns training programs at an FDA-regulated site. It covers what investigators actually look at when they pull training records, the failure patterns that keep showing up, and where AI-augmented quality systems are starting to change what’s operationally possible.
What 21 CFR 211.25 Actually Requires (And What It Doesn’t Say)
The regulation is deceptively short. Each person engaged in manufacturing, processing, packing, or holding a drug product must have “education, training, and experience, or any combination thereof, to enable that person to perform the assigned functions.” It also mandates training in current GMP regulations and in the specific written procedures applicable to the employee’s role.
What 21 CFR 211.25 deliberately leaves open is format, frequency, and platform. FDA doesn’t mandate e-learning modules or annual refreshers. That flexibility sounds helpful until your investigator starts asking questions you can’t answer from your records alone.
Here’s what inspectors actually look for when they request training files during a routine surveillance inspection or a for-cause visit:
- Role-specificity. Not department-wide training, but training tied to the individual SOPs that govern the employee’s actual tasks. A training log that shows “GMP Fundamentals — completed” is not the same as “SOP-QC-041 v4.2 — completed by [name], [date].”
- Timing relative to task performance. Training must precede the first execution of a procedure. This is non-negotiable. If an analyst ran HPLC tests before completing training on the applicable analytical method SOP, that’s a 211.25 observation.
- Revision-linked training. Every time an SOP is revised, every employee whose role requires that procedure must complete training on the updated version — before performing tasks governed by the revision.
- Effectiveness verification. FDA expects more than a signature. For critical operations — sterile fill, in-process testing, environmental monitoring — inspectors look for competency assessment: a quiz result, a supervisor’s documented observation, or a qualification exercise.
- Specialized qualifications. Analysts performing instrumental methods need qualification records under the applicable analytical procedures. These live in a different file than general training logs, and investigators request them separately.
The medical device equivalent sits in 21 CFR 820.25 (and, under the updated QMSR framework aligning with ISO 13485, in those corresponding quality system requirements). The structure is parallel, the expectation is the same: documented evidence that the person performing the task had the training to do it correctly.
The Failure Pattern FDA Keeps Finding
The most common training compliance failure isn’t missing records. It’s records that can’t be reconstructed quickly enough to tell a coherent story under inspection conditions.
Here’s the scenario that plays out more often than any quality professional wants to admit:
A site issues a revised in-process testing SOP — call it SOP-QC-017, revision 5. The document controller marks it effective in the QMS on April 7. Training is scheduled for the following week. But due to a compressed production schedule, two analysts on the B-shift don’t complete training until April 28. In those 21 days, they ran 8 in-process tests under the revised procedure.
When an investigator reviews the batch records for those lots and cross-references them against training completion dates, those 21 days are visible. The observation writes itself: personnel performed operations in accordance with procedures they had not been trained on. That’s a 21 CFR 211.25 citation, potentially a 21 CFR 211.100(b) citation for failure to follow written procedures, and depending on the disposition status of those batches, possibly a 211.192 production record review failure — all from a single missed training window.
The facility knew training was required. The QA team scheduled it. But the control that linked SOP revision to mandatory training completion — before the next execution of that procedure — wasn’t systematic. It relied on supervisors catching it, and supervisors were managing a production crunch.
This is the structural vulnerability that regulatory compliance consulting engagements surface repeatedly: a training program that functions well under normal operating conditions but breaks down under exactly the kind of pressure that also produces deviations.
What a Training Compliance Audit Actually Examines
Strong regulatory compliance consulting work on training programs goes considerably deeper than record completeness. A rigorous assessment works through five layers:
1. Curriculum architecture. Does your training structure map to actual task risk, or to organizational convenience? A cross-trained employee who performs both raw material sampling and visual inspection needs training records that explicitly cover both functions. Siloing training by department — rather than by task — creates gaps that are invisible until an investigator asks about a specific batch record.
2. Procedure linkage. Every SOP should generate an explicit list of roles required to train on it. If your document management system can’t produce that matrix automatically, you’re relying on human memory to maintain it. Human memory doesn’t hold up when an investigator asks which of your 47 analysts were required to train on SOP-MFG-023 and whether all of them did so before the effective date.
3. Effectiveness verification standards. Define what “trained” means for each category of procedure. For low-risk administrative SOPs, a read-and-understood signature may be defensible. For analytical methods, sterile processing procedures, or operations involving controlled substances, FDA expects observed competency demonstration. That expectation should be encoded in your training SOP — not left to individual managers to interpret.
4. Retraining triggers. Beyond SOP revisions, what events mandate retraining? Deviation involvement where a training gap was a contributing factor should. Role changes should. Extended leave of 90 days or more should. Most facilities define this in their training procedure; fewer enforce it through systematic controls rather than supervisor judgment.
5. Cross-functional and contingent worker coverage. Contract workers, temporary staff, and personnel borrowed from other sites during surge periods are the most common blind spots in training audits. FDA does not distinguish between full-time employees and contractors when assessing training compliance. If a contractor ran a process, they needed training records on that process.
How AI Is Closing the Gap

What Inspection Day Actually Looks Like
FDA investigators requesting training records during an inspection typically work from a specific batch or process under review. They ask for the training records of every person who signed a batch record, executed an in-process test, performed a line clearance, or approved a deviation associated with the product in question. That request can cover 15 to 30 individuals across multiple shifts and functional areas.
Sites that maintain training records in disconnected systems — a paper log here, an LMS export there, individual manager folders somewhere else — routinely produce incomplete records under that time pressure. Not because training didn’t occur. Because the records aren’t retrievable in the unified, cross-referenced format the request implies.
That retrieval failure creates its own problem: an investigator who can’t confirm training was completed has to assume it wasn’t. The observation gets written based on absence of evidence, even when the evidence exists somewhere in the system.
